Thursday, Jun. 26, 2008; 3:27 AM

LOS ANGELES (Reuters) - Actress Heather Locklear, famous for roles on television dramas "Dynasty" and "Melrose Place," has checked into an Arizona medical facility for psychological treatment, her spokeswoman said on Tuesday.

"Heather has been dealing with anxiety and depression," Locklear's spokeswoman Cece Yorke said in a statement to Reuters. "She requested an in depth evaluation of her medication and entered into a medical facility for proper diagnosis and treatment."

Yorke said the matter was confidential and no other statements would be released. The story was first reported in People magazine.

Locklear, 46, gained TV stardom for her role as Sammy Jo on the popular 1980's soap drama "Dynasty." Her fame grew when she joined the cast of "Melrose Place" in 1993, starring as vixen Amanda Woodward.

She has been married and divorced twice, once with Motley Crue drummer Tommy Lee and a second time with Bon Jovi guitarist Richie Sambora. She has one daughter from her marriage with Sambora.
